I received a settlement Notice in the mail:
If you received a settlement Notice in the mail, a Claim Form will then be sent to you automatically. At that time, the Claim Form will also be posted on this website and available by calling the toll free number.
I did NOT receive a settlement Notice in the mail:
If you did not receive a settlement Notice in the mail, you may use the form below to pre-register to receive a Claim Form. You do not have to pre-register but doing so may be helpful, and does not impact your rights in this case. If you would like to submit any additional information to the Class Administrator at this time, this is optional and you may send an email to info@PaymentCardSettlement.com.
Changes in Mailing Address, etc.:
If your mailing address or other information has changed, you may use this form to provide updated information to the Class Administrator.
Proofs of Authority:
Please do not use this form to submit any proof of authority (POA) to file a claim for another person or entity. Such POA requests are not being accepted at this time.
